North Coast Health receives grant to improve patient outcomes

North Coast Health has received a grant from the CVS Health Foundation in partnership with the National Association of Free & Charitable Clinics. The grant will provide more access to health care and improve patient outcomes in the Greater Western Cleveland community. North Coast Health is one of only 34 clinics nationwide to receive CVS Health Foundation funding this year.

The $35,000 grant has been awarded as part of the CVS Health Foundation’s commitment to address the need for more accessible, coordinated health care in our local communities. The grant will help North Coast Health support Improving Outcomes for Patients with Diabetes, Hypertension and Depression.

“The grant we are receiving from the CVS Health Foundation will expand our efforts to reduce overall health care costs and improve patient care outcomes for those in need,” said Gina Gavlak, North Coast Health President & CEO. “We are honored to receive this grant which will help ensure that we can support underserved people in the Greater Western Cleveland community who are looking for help managing diabetes, hypertension and depression."

Grants awarded to free clinics nationwide in partnership with the National Association of Free & Charitable Clinics have allowed organizations to increase operating hours and the number of patient visits, resulting in a decrease in the number of emergency department visits and improved patient compliance.

The free clinic grants are part of the CVS Health Foundation’s $5 million, multi-year commitment to supporting community health and expanding access to quality health care nationwide through partnerships with the National Association of Free & Charitable Clinics and the National Association of Community Health Centers. North Coast Health is proud to be a partner in these efforts.
